Protect Yourself! An Advance Health Care Directive (AHCD) speaks for you when you cannot speak for yourself. An Advance Health Care Directive (AHCD) lets you have a say about how you want to be cared for if you cannot speak for yourself. Learn how to prepare your AHCD at this FREE presentation. Topics covered include: • What AHCDs are, why you should have one, and how to complete the forms • Appointing an agent, end-of-life decision making and POLST forms • Referrals for other planning tools such as Wills and Durable Power of Attorney for Finance • Suggestions and resources for preparedness Drop in program. No registration. Presented by the Health Insurance Counseling and Advocacy Program (HICAP) of Legal Assistance for Seniors. Library staff may take photos, videos, or audio recordings at our events.
